This explains a lot. When I attended the Federalist Society National Lawyer’s Convention in 2008, AG Mukasey gave the keynote address during the dinner. He collapsed (still not sure why). It felt like it took at least 15 minutes for an ambulance to show up. Mind you, we were in the Marriot Wardman, he had a team of secret service, and there were probably 4,000 of the most important lawyers in the United States in the room. Indeed, when seconds count, the government is minutes away.
